The Inclusive Health Partnership is committed to overcoming health inequality by providing high quality, integrated and holistic care. In 2024-25, the Inclusive Health and Wellness Hub continued to provide General Practice, nursing, dental and wellness programs, vaccination clinics, outreach clinics and commenced a social Work in General Practice Pilot Program.
