Homeless to Home Healthcare Network

The Homeless to Home Healthcare Network aims to:

  • Promote a person centred, trauma sensitive, recovery oriented approach to support the well-being of people moving from homelessness to home
  • Improve access and equity for people moving from homelessness to home to appropriate  healthcare to promote wellbeing and quality of life
  • Promote collaboration so as to sustain connections between housing options, support services and healthcare across government and non government service systems
  •  Linking primary healthcare, mental health, drug and alcohol services, and acute healthcare services (where appropriate) through coordination and planning
  • Facilitate the inclusion of alternative healthcare practitioners in the provision of services to people
  • Research and advocate for new models of housing plus healthcare service provision to meet the identified needs of people and gaps in services system.
