Homeless to Home Healthcare Network
The Homeless to Home Healthcare Network aims to:
- Promote a person centred, trauma sensitive, recovery oriented approach to support the well-being of people moving from homelessness to home
- Improve access and equity for people moving from homelessness to home to appropriate healthcare to promote wellbeing and quality of life
- Promote collaboration so as to sustain connections between housing options, support services and healthcare across government and non government service systems
- Linking primary healthcare, mental health, drug and alcohol services, and acute healthcare services (where appropriate) through coordination and planning
- Facilitate the inclusion of alternative healthcare practitioners in the provision of services to people
- Research and advocate for new models of housing plus healthcare service provision to meet the identified needs of people and gaps in services system.
